Died in River of Flame on Hell, picked up my body and my Crescent Moon phase and HoZ were gone.. nowhere to be found. Has this happened to anyone else?

i just killed ubers and i picked up torch and died and it disappeared so.. nothing anyone can do right?

I lost a set of gloves after trading with someone. I exited my game, traded with him, entered my own game, and my gloves were gone.
