Died and lost items

Died in

River of Flame on Hell, picked up my body and my Crescent Moon phase and HoZ were gone.. nowhere to be found. Has this happened to anyone else?
Blizz doesnt restore items so GG for me I guess :/ bummer

Did you equip the Hammer to do Hell Forge Quest and then died with the Hammer in your hand?

Going to stretch here and say he quick exited and lost them while they were on the Ground.

i just killed ubers and i picked up torch and died and it disappeared so.. nothing anyone can do right?

woah really? i keep seeing a lot of random stories like you two on reddit as well. this is very concerning.

I lost a set of gloves after trading with someone. I exited my game, traded with him, entered my own game, and my gloves were gone.
