The Book of Skill is dropped by Radament once per difficulty whilst on the Radament's Lair quest.
You may right click the book to earn 1 skill point. One book may be obtained per difficulty, meaning you can have a total of +3 to all skills if you make it past Sewers Level 2 in Hell difficulty.

This extremely valuable, once-per-difficulty reward can be missed if the Radament's Lair quest is completed before the tome is obtained. See this thread for examples of how this might occur.
If this occurs, do not speak to Atma to end the quest. Slay Radament again with the quest still active, and a new tome should drop. If the quest is completed, multiplayer characters can obtain a missed tome by joining another player as they defeat Radament to complete the quest at the appropriate difficulty.
