You mean the little diablo head icon to the left of the 1, right of the game name? That's the indicator telling you that Terror zones are enabled in that game.
Secret level probably (educated guess) refers to him doing a cow run.

Sorry, I should elaborate. I mean that the game is usually open but has been open for nearly a week in hardcore mode. Whenever the game is remade, it stay open again for a very lengthy time. Usually another week.
