Yes I have, in Act 1 of hell. It blocked the entrance into Cold Plains coming from Blood Moor.
I've seen a couple of these since D2R. I never remember seeing any prior to it, though.

Lots of fun to be had when going melee against a mob of these with LE + Stone skin. On a positive note, they give great experience.
Haha yes, they were very satisfying to kill. Nothing like a monster that can't move or run away
Quite rare indeed, I think I've seen it once myself in D2R, and also saw a streamer run into it once as well. Preeeeetty sure I saw it in old D2 as well, like once or twice.
