Interesting, I've never had the Herald actually spawn and not be able to find him. Is it possible you killed him without noticing? Interesting. Last night I was doing Palace Cellar and drew the Herald at xx:59, 1 minute before TZ change, and there ended up being no Herald.
I did notice the zone staying terrorized. It's very obvious if you happen to be doing early act normal TZ because super healing/mana potions continue to drop.
It was 110% "the Herald is hunting you". Can't miss the sound when playing with headphones.

I'm still inclined to assume that the handful of "it never spawned!" stories are any of...
- Only got ire but no hunting and misunderstood that ire alone doesn't spawn anything
- Didn't search everywhere and simply missed it
- Killed the Herald without noticing (some of them do die in a single hit after all..especially easy to miss if you run a Dream or Dragon build, a multi-shot zon or some other build that can kill stuff off-screen on a regular basis)

I don't think so considering it's so small of an area. My warlock does decent damage, but it's not crazy considering I'm using Blade of alibaba for the MF. 9-13K is ok, but I can't say I've killed any heralds w/o knowing I was engaging one.
I just terrorized the entire game yesterday from Act I to Act V and got 18 T5 heralds. Obviously, no sunder from them.fredkid wrote: 4 days agoSo... I got almost double the average

I found one Bone Break however, after 176 heralds in total, 79 of Terror (T5). The sunder dropped from a T4 but a T1 gave me a Tyrael's Might.

Not if they were the last person in the game. These are some interesting ideas, I'll need to do some science on this and get back to you.
Pretty sure that was never accurate or got hotfixed along the way.
Case and point:
I was running some TZs (2 days ago I believe it was) in a private (Game2903492034whatever) game and had an ire for T3 ready and waiting to be triggered when a trade wanted to be completed.
Didn't want to lose the game/progress.
-> Asked trader if he could join hell
-> Switched my character to the one with the thing to be traded (=game was empty until I rejoined)
-> Invited trader afterwards, trader joined, trade completed
-> Trader left
-> I left to get my original char back to keep going (=game was empty a 2nd time)
-> Continued, got the Herald about a zone in (without another ire in between)
So after the game was completely empty not just once but twice, the ire was still preserved and turned into the same T3 Herald it would've triggered if I had just kept going.
9-13k is more than enough to accidentally insta-kill some Herald types without noticing. Even T5.
So yeah, could've still just been an accidental miss.

You can re-join private online games (non-lobby) too 
If you press "P" (to show the login/password of the game)
In private game will show a random game name and you can't see the password.
Buy you can leave and re-join the game using that "game name" as login and password
(the issue is that you cannot copy/paste the random name...)
You can use that if you face a monster too strong for your current character to change to another stronger character
