I keep a manual track (I made a "super spreadsheet" on google docs )
This seems to be very time consuming and pretty annoying.
When you find something you have to pause the game, enter the spreadsheet and edit it then back to the game.
I'm for detailed statistics but I find it very complicated. It was very nice to have a counter in that app to record the total numbers of drops.
Strictly speaking, I do not take any notes about my drops, but on top of my head i would say that it's Nagelring for uniques, and cathan for sets. Hate both of them
I only track "new" unique/set drops, or update the register when a better version drops. Once I find something in its perfect form, I stop picking it up again.
The usual suspects would be the damn angelic ring, haemosu's armor, Aldur's weapon, and the low level unique bows.

Can you send me the file or link to see it?
I also started an Excel spreadsheet to keep track of grail for my last character (Barbarian). If you want it I can give you the file.
The grand Holy Grail is at 99% (only 8 items left).
The Barbarian just finished Nightmare and found a total of 244 items (containing duplicates). Just 146 are of single found, the rest are duplicates of those 146 but not a new entry for the Grail.
I sacrificed damage for magic find, so the killing speed is outrageous slow. Now is at 803 MF but I think I have to lower it for Hell.
I play with double Isted Gulls (upgraded twice to Bone Knife) and all the MF gear.
To be on topic, the most found one is Isenhart's Case, with 10 items found.

After a 20 year break from D2 I started playing D2R in 2024 with my sons (9 and 10 years old)

Isenfart's set is the most annoying items in the game. I just can't help but swear every time one drops (which means there is usually a lot of swearing)

They REALLY should buff it at least a little. Give each piece an extra bonus and then either 1 bonus for each extra set piece used or +1 bonus for every set piece beyond 2. Or heck, ALL those, because it's a really LAME set as it is. The only part that's useful is the sword because of its massive and reliably high damage from level 8, but even that quickly becomes obsolete.
Same with most of the other lowend sets and even some of the highend sets. It's so sad that there's so much potential wasted with all those inferior sets.
