Prior to Patch 2.6, Blizzard Devs created a mechanical issue with their implementation of a permanent Quest Loot with the release of D2R that triggered overriding issues when Andariel was Terrorized. This ultimately restricted her treasure class tables from upgrading. I covered this in the video below:
However, as of patch 2.6, thanks for a lot of the visibility we surfaced, this was fixed. When Andariel is Terrorized she now properly upgrades!
