As a writer it is difficult not to criticize lazy attention to detail. Cain describes the Malus as being created by ancient Horadric mages during the Sin War. Later (in D3 naturally),

Tyrael claims to have founded the Horadrim 260 years prior to that game's events, but this obviously does not jive with the Sin War reference, which is around a thousand years ago.

A great deal of Act 2 of D2 does not align with that particular, unnecessary oops-redaction.
