I found a cracked Berserker Axe in a chest in cow level. Trying to roll 5 sockets. What is the proper repair recipe and will that help me get a 5os or will it always be 4 from cubing it if I go that route and 6 from Larz

Using the following recipe, will result in the item having an iLvl = 1. A Berserker Axe with a iLvl of 1 can roll up to 4 sockets, even through lazurk.

Tangentially related, this is one of the reasons why non-superior zero-socket elite bases are hard to sell. The buyer doesn't know the item's iLvl. Thusly not knowing the socket possibilities.
So if you use cube to fix a bad quality item into a normal item, you cannot use Larzuk to max the socket ? What about the recipe to add socket ? Does the item Will have the same issue or you can get the max socket possible ?

An item's ilvl determines the max sockets for that particular item. Even though a berserker can have a max of 6 sockets, if the ilvl is 1 (which is what you get when cubing a low quality item) the new max is 4 sockets. So if you lazurk this one you're guaranteed to get 4os. If you cube you'll get a roll of between 1-4.
Is the dice still 1-6 so 50% to get 4 os then 17% to get lower because 4 os is max for the BA or 1-4 so 25% to get 4 os ?

Moreover, the 1-6 roll is made regardless of the base item and its ilvl. For example, a repaired Monarch, limited to 4 sockets by base item and to 3 by low ilvl has 1/6 chance to get 1 or 2 sockets and 4/6 to get 3, not uniformly 1/3 each.
Maxroll has a table with all the possible odds for 2-6 sockets.

Unless one wants to take advantage of the charges on it (level 18 Heart of Wolverine). I always made Oath in ethereal bases myself but I heard of this angle fairly recently, someone found the HoW charges useful.
