but have an endgame item on Exceptional and Elite variants is already a great achievement 
(don't think there are many cases)
and ... if you count Set items, you can count Death's Guard for the Normal variant (not a endgame item, but with Death's Hand can carry your attack build until you start to replace equipment for BIS)
