Did one quick TZ Baal run by myself last night and Baal dropped an Arreat's and a high roll Eth Death's Fathom in the same drop. Couldn't believe it.
I found an ethereal Herald Of Zakarum. I decided to up it and put a Zod in it. Should slap some ubers around good.

Edit: 1. A weapon like Grief which has + damage. 2. Non-weapon enhanced damage %, strength %, aura skill level. These factors can also affect Smite damage.
