I've taken a bit of time off after launch and am just starting back up. Briefly looking through the trade board I see that there are trades for pretty decent items just asking for Perfect Gems. Never seen that before but it seems common, did something change in the last couple years with one of the new patches that added value to gems?

Its been long enough in D2R that nearly every item, that isn't BiS, is very hard to sell. Most who buy non-bis items are people just starting out, and they usually don't have much to trade except Perfect Gems. Those Perfect Gems are used to reroll grand charms hoping to get skillers with a good suffix (life, fhr, runwalk, etc). Like what I JUST actually did. I got a pcomb with 38 life.
All trades can be accomplished via PC or xbox. All reasonable offers will be considered and probably accepted.
Bots happened. Thousands upon thousands, farming items 24/7. Also, NL in particular was bound to get lower and lower values over time, as it's the "eternal" realm that never resets.
Nothing has added value to gems specifically. Item values have gone down in general (with the exception of perfect and near-perfect rolls), rune values have gone down, rune relative values have been "crushed" (runes are closer to each other in terms of relative value than they used to be, especially higher runes), perfect amethysts in particular are used a lot as smaller currency than runes (think of it as cents to dollars).
Early ladder still sees something close to the old days values (but still not quite), but they quickly drop over the season and converge around NL prices eventually, which are much lower overall compared to 2Ā½ years ago (first ladder season was 2Ā½ years ago). For NL specifically it's just natural as it never resets, it just has an ever increasing amount of available items.

Perfect Gems are used to reroll stats on high level magic Grand Charms. A favorite outcome would be a +skiller with 40-45 life.
Perfect Gems such as Amethysts and Rubies should be separated from other P-gems because Rubies are used for crafting blood items, and P Amys which are the most valuable of P Gems have a number of uses. Amethysts have several different uses such as adding sockets to normal quality items which is a bit of a gamble since the number of rolled sockets can be any number from an items minimum and maximum .
And yes, while other items have generally dopped a fair bit in value over time, the value of Perfect Gems are pretty stable as there is and will always be a need for them, as they are used in crafting and rerolling. They are constantly consumed, and there will always be a need for more.
