Description
Baal is the final boss in the game, culminating Act 5 and is found in the Worldstone Chamber.
He is often farmed on magic find runs due to his ease of access and high mlvl/Treasure Class.
Rush Baal and quickly do some damage to him. Don't waste time attacking the "fake" Baal. Static Field and Crushing Blow is very nice for bringing his health down.

Baal was the most brash and reckless of the Prime Evils. After the Dark Exile, he was contained in the Horadrim Tal Rasha and entombed.Centuries later, Diablo freed Baal, who then corrupted the Worldstone to devastating effect for the barbarians who lived near Mount Arreat. The heroes killed Baal shortly afterward

For whatever reason, casting Decrepify on Baal completely breaks his AI, and essentially forces him to stand still. He won't Teleport, or clone himself; making him absurdly easy to kill.
BAAL, Lord of Destruction (Excidium)
"The soldiers of Baal seek the undoing of the universe. They strive for ultimate disorder and destruction, and to this end covet the destruction of all they behold. Order is an abhorrence to them and these creatures are the manifestation of the forces of chaos."
- From the original Diablo manual, 1997

The numbers in the players column are the total number of players in the game (or your players setting in singleplayer) / the number of party members nearby. Listing all possible combinations of player numbers and party members nearby would be a bit too much for this post, but you can check them in the source.
The chances are a bit higher if you do the Eve of Destruction quest.

Fun trivia: in the game files, Baal's id is baalcrab. Crab.
This begs the question, crab, if there was originally planned to be an additional humanoid (non-crab) Baal boss. Crab.

Lore from the LoD manual:
The Coming of the Lord of Destruction
"And a child will cradle Terror in his breast as the heart of man falls under the shadow.
A Wanderer will pass through the ancient lands trailing Chaos in his wake.
The Three Brothers will be reunited as the mortal world trembles before their might.
And so it was foretold that the Three, once reunited, would be shattered again—
And the last of them would set his sights on the holy mount. The warnings held that
Their defeat would be illusory—that the final gambit had yet to be played…"
And now at last the storm surges forth from the southlands, and the hand of Destruction
Reaches out to undo the workings of the Ancients. The tides of Hell surge—ready to smash
Down upon the shores of the mortal world—to drown the guilty and the innocent alike."
