The Countess is well known for her high chances to drop runes. She is often farmed early on in the game in order to collect the runes required for an early runeword such as Stealth, and to bolster rune stocks for future runewords such as Spirit.

The Countess doesn’t really drop up to Lo. While that is technically possible, she never really drops above Ist.
Her drops are split into two types: her special rune drop and her normal drop. Her special rune drop only goes up to Ist. Her normal drop goes up to Lo, but she’s no more likely to drop a Lo rune than any other monster. In fact, she’s quite a bit less likely to drop a Lo rune than random ghost monsters on the same floor as her, since her normal drop tables don’t favour runes that heavily.
Basically, if you really want to find your own Ist rune, countess might be a good place to farm it (although it’s still quite rare). If you want to find your own Lo rune, do Secret Cow Level, Chaos Sanctuary, Arcane Sanctuary or anything else.
My advice is mainly to grind her a couple of times in nightmare. First of all the runs are good xp if you clear out the tower levels. And next to that you are very likely to be able to make Spirit, Stealth, Lore, Rhyme, Ancient's Pledge and Insight (for merc) very early in the game.
The higher runes in her regular drop table are as follows (/players 1):

When looking for those runes it is better to do high-density farming elsewhere (ex: The Secret Cow Level)

For every level of The Countess' Forgotten Tower, the rule is that your character's path through the exit tile is to the LEFT of the entrance tile's orientation.
I farmed Countess for some time on the Hell difficulty. TBH, it was quite rewarding (sorceress). I got 3-4 Ist, 4-5 Mal, plenty of Pul, Um and Lem. Above that I also got 2 x Vex. No Lo though.
