Hello, I was wondering why you never hear about high level grand charms being able to be dropped by Minion of Destruction as well as Diablo, Nihlathak, and Baal.

Former monsters are mlvl 92 and according to Arreat Summit database, and can drop same ilvl grand charms. The of Vita with 41-45 to life is ilvl 91.

Am I missing something? Thanks!

Baal's minions shouldn't be mlvl 92. Their level is adjusted by the area in NM/Hell, so they must be 88. I think Arreat Summit is wrong here.

Only the other three targets you mentioned drop grand charms that can roll all affixes.

So if you find a GC from Baal, do you reroll always? Or do you only reroll based on original grand charm level? I have rerolled about 15-20 times each on two GCs dropped from Baal and have seen nothing close to useful yet, let alone spectacular

Unique monsters and their minions have alvl+3. Super unique monsters including all bosses have pre-determinated monster levels.

This is a common misconception. You are conflating item requirements with ilvl; they are two different things. No matter how many times you reroll a magic item, it's ilvl does not change because you rolled low level affixes and the required level of the item is low. Keep rerolling it until you get at least the 41+ life suffix as you can trade it for an Ist rune (since it is verifiable a " Baal" grand charm).

Baal, Nihlathak and Diablo

If you plan to collect from Nihlathak though, make sure to pay attention as you kill him to be certain he was actually the one dropping it, as opposed to one of the countless minions sitting on top of him.
