Never seen this on PC. Maybe it's a PlayStation bug?
Yes very often since the last patch yesterday on my PS5. Maybe every 30 seconds. My brother doesn't have this happen to him though on his PS5. Hope they fix it soon.

Just some ghost items lying on the floor invisibly (= much more transparent than ethereal). Try to find the spot where the 6 Griffons lie and grab them.
Yes, the game has been running like shit on ps5 since the patch. Ui constantly fading, seeing this same screen every time I exit game, random half second freezes.
Same problem here. It actually led to a hard crash for me already, which I reported via the PS5 report screen. (Was leveling a Warlock, killed an elite pack, than Black screen.)
Its driving me nuts. Might just redownload the game in hopes that it will resolve itself.

This is apparently a widespread issue amongst ps4 and ps5 users since the update. There are some threads on the official forums about this as well

A very quick test with the Horadric Cube says: Bug's gone.
Edit: Just started the game so I cannot tell if anything else has changes, neither visual stuff nor game related updates.
