The ones that comes to my mind:

Tal Rasha's Tomb
Secret cow Level
Worldstone Keep ( Worldstone Keep Level 1, Worldstone Keep Level 2, Worldstone Keep Level 3)
" Nihlathak's dungeon" ( Halls of Anguish, Halls of Pain, Halls of Vaught)
Chaos Sanctuary
Flayer Jungle
