When placed in the inventory (not in Horadric Cube), it reduces all enemies' physical immunity (100%+ physical resistance) to 95% resistance as well as confers -20% to -10% physical resistance to your character.
Physical resistance is listed under advanced stats, if it's higher or lower than zero, and increases (if in negative values) the damage suffered from physical attacks, which is the most of all sources.
Bone Break only drops from champion, superunique, or boss-type monsters in terrorized zones on Hell difficulty with a relative drop chance of 1:17 (on average 1 in 17 sunder charms dropped could be a bone break).

List of physical immune monsters affected by Bone Break. Notably, some unique enemies can have the Stone Skin modifier adding 50% physical resistance on top of what they already possess making them immune as well, eg. Hell Bovine.
Damage Reduced by X% (the ingame wording for the effect "Physical Resist +X%", i.e. the opposite of the penalty imposed by this charm) is rare as compared to elemental resistances. However, Physical Resistance is not subject to a difficulty penalty. This means that Bone Break is not piling onto an already penalized attribute. Damage Reduced by X% sufficient to compensate for Bone Break's penalty will remain sufficient in Hell difficulty.

To make it work for your summons (e.g. wolves, skeletons, Valkyrie), you HAVE to summon them with the charm ON (in inventory).
Then you can put it back in the cube.
This is also true of the Physical damage dealt against Physical Immune targets affected by items such as Atma's Scarab or The Reaper's Toll.
