If you’re looking for added lightning resistance, an Ort rune is the choice for Armors and Helms. However, when it comes to shields, the Perfect Topaz outclasses the Ort rune. Whereas Ort gives 35% lightning resistance in shields, the Perfect Topaz gives 40%.
Put 4 of these in a Monarch and you’ll have a cheap-ish 160% lightning resistance boost.
If you’re a paladin needing a boost, 4 of these in a paladin only shield with a base mod of +45 all res will get you +205 lightning resistance from a single item. Not a bad investment.
