No, only the physical javelin can trigger it (same as life stolen per hit).

Lightning Fury has two effects - the physical projetile (the actual javelin being thrown), and the skill effect. The physical portion, the javelin being thrown, isn't replaced by the skill effect, it's still there. It will apply all the physical effects that a regular jab or throw would (crushing blow, deadly strike, life stolen per hit, etc).

No, the only spell that receives a damage bonus from

Concentration is

Blessed Hammer. Other than that, only physical attacks (with a weapon) receive a bonus from

Concentration.

Correct.

For other classes wearing Enigma, the

Teleport level is only boosted by "+X to All Skills", but for a Sorceress the level is boosted also by "+X to Sorceress Skill Levels" as well as "+X to Lightning Skills (Sorceress Only)".

Yes. Wont change it being ethereal though, only the affixes (which are dependent on the iLvL).

You use a drop calculator. One of the best ones in D2R is Nishi's (afaik it's the only one that properly handles TZ):

For example, to find

Andariel's Visage with 0% MF at p1 in TZ at character level 94 (TZ level 96, the max):

You see odds for full area clears on the left vs individual bosses or super unique packs on the right.
Just adjust the values accordingly to match your char's stats. You can also toggle monster types to for example exclude normal mobs, and more.

Andariel's Visage is Hell

Baal IIRC.

Generally for farming set and unique items the best strategy is player count of 1-3 with priority on high MF, skip past mobs as much as you can and go straight for the boss or super unique of the pack. Higher player count does increase the drop chances for sets and uniques, but generally speaking higher MF w/ lower player count wins vs High player count w/ less MF in this case (as opposed to farming runes and bases where the opposite is true).
