If there are fewer than five monsters remaining in The Den of Evil and a Fallen Shaman resurrects enough Fallen to increase the number above 5, the quest window will display, Monsters remaining: 666.
If you complete the quest Den of Evil, the area name change from The Den of Evil to The Cave.
When starting act 1 leave a Town Portal by the Den of Evil, run to the Cold Plains waypoint, go to town to sell items and buy a few Stamina potions, and take the Town portal back. Way faster and more efficient.
