Are you sure you didn’t take damage at the same time (which nullified some of the healing), and that you didn’t actually drink a regular
Rejuvenation Potion
(which heals 35%, if I recall correctly)? Regular rejuvs can sneakily make it into your
Belt
if you accidentally click on one on the Ground while there’s free spade in your
Belt
. I know I’ve clicked regular rejuvs into my
Belt
by mistake many times.

I'm in CET (Central European Time), so that's UTC+1 normally and UTC+2 during DST.
My profile says Ladder, but I play both Ladder and Non-Ladder.

If it happened once or twice, i could possibly see that i might have missed like that, but it happened several more times, when i had nothing but full rejuvs, and monsters that caused it were dead or i had retreated behind cover.

Not impossible, but i've never seen it happen before, and i had like over 40 level 75+ chars in original D2...

There is a thing called “Chance of Double Heal” in this game. The formulas are as follows:

Chance of Double Heal for Vitality up to 200 if Vitality is even: (Vit - 2) / 4
Chance of Double Heal for Vitality up to 200 if Vitality is odd: (Vit - 1) * (Vit - 1) / (4 * Vit)
Chance of Double Heal for 200 Vitality and up: 100 * (Vit - 101) / Vit

I would guess that transparent graphic is the indicator of double heal, and the healing stop at the real heal point. Hope that make sense.

I was considering that, but I don't think that's the case here.
  • - potions critting previews their full healing potential correctly on my chars
    - the numbers I ran (in my previous post) would indicate an additional 50% healing, not double

Didn't much testing yet but I have noticed that that after some equipment swaps it no longer does that bug thingy.
One of the items swapped was
Manald Heal
.
That makes me wonder if replenish life modifier does something weird with potion regen.
