Hearth, as the name implies, protects the wearer from cold damage and freezing.
This rune word can only be MADE on ladder, but you can find these in the wild being used & traded on non-ladder because whenever a ladder season ends, all characters & items from that ladder season, including any Hearth and other ladder-only rune words that were created during that ladder season, will transfer over to non-ladder. The ones you come across on non-ladder were created during a previous ladder season.
Despite paling against its biggest competitor, Cure, this runeword has some valid usecases in ladder mid-game:
- On Travincal Horker, as a cheap replacement for the Cham'ed helmet like Arreat's Face, because it's a source of Cannot be frozen that does not shatter enemies. Best base would be +3 Find Item +3 Battle Orders
- On Blizzard/ Frozen Orb Sorc, Frostmaiden Ama and Wind Druid to compensate the negative resistance of Cold Rupture, usualy being far from perfect when found early in the ladder
