Even though Blizzard kept denying it, there were persistent rumours of secret level accessed by clicking a certain cow in Tristram. The rumour was so widespread that they actually created The Secret Cow Level as a joke in D2.
The joke continues to this day with Blizzard continuing to "deny" its existence.

There is no cow level.
If ever there was a cow level, it would have been removed long ago for the sake of the environment as Blizz is unwilling to throw any money at D2 to improve the cows' diets and lower their emissions.

By the way, did they patch out the symbols in the life bar of monsters in the TZ? I thought there was a diablo head in each of them. This season i saw none, just normal life bars like no TZ monster.

Let me guess, you're not using the US/English version of D2, right?
Apparently, there's some bug that may (for some people) remove the terrorized life bar icons in translated versions of the game. They're still terrorized and the loot is still Terror-loot (both confirmed), you simply don't see the icons. You're not the first to have that problem.
