This has happened to me twice now in D2R. Never in old D2.

there has occasionally been glitches where the icon for the next level wont appear on map but in game actually exists.

it's the little unexplored section on the bottom right, there is likely a narrow trail that opens up and then turns down so it doesn't display, I've had that happen to me a few times and have the entrance really close to the start of lvl 1.
All map entrance tiles only have one exit. That has never happened to me. If you play the game enough however, you'll know all the existing map tiles and how they connect.
