To be fair, there's a ton of normal and even solid/good/valuable drops in here that are being moaned about as fail drops. Seems the bar for what constitutes a fail drop for a modern day gamer is basically anything that isn't a perfectly rolled BiS.
Second, the elite version, the Vortex Shield is considered BiS for some Paladin that go with maximum defense (the Defiance aura helping a lot here).
Let me put it that way: are you happy to drop a Cham when you could have drop a Jah instead? That the same thing here, it like a compensation price. Useable, but not the Best.

Sorry but that right there is exactly what's wrong with modern day gamers and their being used to having everything served to them on a silver platter.
If getting one of the rarest runes out there that many people will in fact never see and that even if you don't need it easily converts to Ohm value is a fail drop...you really need to check your expectations.

Well taken in a vacuum, it look like a bad example...
But let me put it this way:
The fact that in two 6 months periods playing D2 heavily, I found 3 Cham and not a single Jah keeps remembering me an half-joke half-truce,
that someone said while explaining loot drop mechanics with headache inducing details, in another forum, in another language, a long time ago: "In summary, A Cham is a Jah that failed to fail"
Pretty sure that not an uncommon story, to drop a Cham when all you need is a Ber or a Jah... Cham and I seems to be cursed at this point and I totally expect my next Sur and above HR to be another Cham =D
That explains my example here. I assumed the Cham market value by itself would justify the parallel.
Also, I didn't expect anything in term of loot from this game, and I think most long term player dont, the loots are so scarces that would be silly.
It just taking a long time to selfmake an entirely selfdrop Enigma at this point (I prefer to selfdrop and selfmake all my gear, so I'm less affected by probable dupes and feel more 'accomplished').
Better take it as a joke, right?

It's rather a fail in term of opportunity, than a fail in term of droped item: an ethereal Vortex Shield with 45 AR 's price is an order of magnitude higher.
That "So close!", like the author said, to be BiS base for Exile.
That definitively a keep, but when you put an Ohm, a Vex and an Ist, people really do try to get the best base possible (like making Last Wish in a 15% ed PB).
Also, if that not your first Shako, an ethereal Shako is not bad on a MF merc ; and a selfdrop perf bad Anni is quite a good twisted token of bad luck. I would keep both too.
Yeah, still for me failed drop would be an EthVortex Shield, 4 os, with +1 all res.
If you know what i mean.
You can make a slightly worse Exile with still a good base with 45allres, but it would be just a fail to make it in vortex, with 1 allres.
It's like a 1 os Superior Eth tresher 15 ED. 1st thing you see is "NIIIIIICE.... oh... wait..."
