A4 was a little rushed to make the Christmas season crunch!
This used to be a big deal for game studios before digital distribution was a thing. Your deadline was basically Summer for distribution to big box stores. Thus LOD basically completed the game about a year later as that content was already mostly designed and planned but just not ready for release initially. They kept patching and adding features even after Blizzard North was shutdown in mid 2000s but it slowed down a lot. That is until recently with D2R so who knows what might be next?
I would not expect it to be very fast after transitions from Condor (D1) -> Blizzard North (D2 & LoD) -> Blizzard (D3) -> Activision Blizzard (D4) -> Blizzard Albany (Vicarious Visions D2R) -> Microsoft (D2R: RotW) -> TBD? ... every M&A adds some amount of confusion into the mix!
I’d personally prefer a new Act 6 rather than expanding Act 4. A new area with its own atmosphere, bosses, quests and loot would feel much more substantial, while Act 4 could always get a few additional quests or zones as part of the same update.
That said, I’d be happy with anything that adds meaningful endgame content without messing too much with the classic feel of the game. A new act that actually feels as memorable as A2 or A3 would be pretty awesome.
What could or should the next expansion be, fleshing out A4 to the level of the rest of the acts or a Brand new A6 or something else altogether?
I think fleshing out Act 4 would be the bare minimum any full expansion should feature. It should have six more zones at least with a waypoint each, and three new quests (provided by Halbu and Jamella, or new NPCs), AND they could expand the Pandemonium Fortress too to make it feel more alive (it's the smallest 'town' and it feels the emptiest; but then again it's also not a mortal realm so they shouldn't go overboard either). The tileset alone is really nice but we don't see it beyond that town area. Hell they could make a dungeon out of that tileset and it would be cool. I'd also want a new class and an Act 6 (or equivalent) as well as a balance pass on existing classes and items. Revamping the mercenaries would also be sick. Give them a skill tree and more item slots for instance. They should feel more like adventuring companions imo.

Plot - Diablo originally intended to create a larger challenge in hell to stop the hero, but ran out of time when the St. Nikolas battles him and delays him while the hero is travelling towards hell.
A corrupted and obsessed follower of diablo and the prime evils gets a magic time Stone and runs time backwards to go back in time to when he was younger to create new challenges to prevent diablo from being defeated
