Apocalypse is massive and works over most obstacles like chests, bushes, water etc. It can also work through walls if you send a demon over the wall with Death Mark.
I noticed that I couldn't put down Apocalypse over certain walls or slopes in Act 5 Frigid Highlands. However, if I sent my demon over to the other side of the wall/slope I could put down Apocalypse at that location.
Basically the demon is a stand in for you.
