Graphic-wise, they're completely normal elite monsters with a pack of minions, the only difference is the Herald itself has a purple effect floating above their head. And as far as I've seen they always have an aura (Concentration, Thorns, Vigor...!)

Yes, three in a row during separate TZ Baal runs. All were in the Throne of Destruction. They were just regular level monsters with swirling purple halos above them.
I saw one just by chance in the recent Cow Tz. In other TZs I'd received the message the Herald Is hunting you (Frozen Tundra). I looked and cleaned the entire lvl but I saw no Herald, the same happened in the triple Kurast TZ, for Lower Kurast. Again I cleaned everything and again I saw nothing, and I decided the problem was probably me. So when the "you're being hunted" message popped up during cows, I took my time and went carefully. Moments later I noticed that one of the harder to kill cows now nearly dead had the Herald label. It occurred to me that I was killing them too fast to identify them. So I'll try to not kill them too quickly and maybe I'll be able to get a screenshot.
I did take a screenshot of the message I got in the triple Kurast TZ.
I encountered another. I made a video of it, but as soon as I got the notification the game began dropping frames, and one the Herald made its appearance the frame rate was so slow that the ending is painful to watch. I did however find a decent frame for a screenshot but with the map showing as well. This time the Herald appeared as one of the carrion birds.

It would be a more sinister element if the Herald did actually chase after you but they seem to be stationary fixed targets within their elite packs. So far none have been of any real difficulty, although I hear that scales with each passing encounter. I've yet to see any proof.
This is a short video of TZ Frigid Highlands. Yes I stupidly stood there taking hits from the Herald of Terror just get this video, and no I didn't die, though I probably should have.
I thought about something and did some testing (only one testing, actually). Hear me out:
See how all our Screenshots and videos show a monster named "Herald of FRIGHT"? Fright is not Terror...
So, I terrorized Act 1 and played through some of it killing elites/champions, got 3 Heralds. Then I terrorized Act 2 in the same game and continued playing... when I reached my 5th Herald, it was a Herald of Terror, and boy was he tanky and dangerous! Hit like a brick, one-shoting the merc several times, almost one-shoting me at 2.3k life (!). I suppose he has HUGE Magic Resistance cause my Blessed Hammers were just tickling him, it took a lot of hammering (I'm on /players7), much more than Act Bosses.
My theory is: THIS is the only Herald which can drop the new Latent Sunder Charms... though he was stingy with me and only dropped rares!
If that's right, then you have to keep spawning Heralds of Fright until you get the 5th Herald in the same game, which will be a Herald of Terror.
I can provide a screenshot later, I'm on my phone now.
Oh, and I forgot to mention: I'm still playing TZs in that same game, and haven't spawned any Herald after meeting the Herald of Terror... perhaps you can only spawn one HoT per game? I'll keep playing to see if this holds true.
I saw a MrLlama video where he explained that there are five levels (good job reaching the last one!) of heralds. They have to be spawned in the same game. Resets each game.
