I think the secret is TZs. My drops in TZs are always way better than in normal areas. Completely biased/cherry-picked evidence of course.
It's a fact

TZ zones have a level equal to clvl + 2 (or the original level of the zone if it is higher), up to L96 on hell (don't remember the max level on normal and NM...)
As long as the TZ level is higher than the original level, the drops are better (monster can drop higher level items/affixes and have better drop tables)
In special:
if TZ level is 87+ all monster can drop any set/unique item
if TZ level is 91+ all monster can drop grand charm of vita with 41+ life
(and remember that the max level a normal zone may have is 85)

Yeah I knew that but wasn't sure if it really applied to runes as well. But I find 90ish% of HRs while doing the better TZ zones (pit/tunnels/trav/cs/wsk).
Edit: I also find if you go back to a character you haven't played in a while (3+days) you're almost guaranteed to get a nice drop whether runes or not. Very tinfoil hat of course, as I doubt there is any correlation and it's just personal bias, but I wouldn't be surprised if there is some underlying code or something that fixes drops to keep you engaged (mainly because Activision does this with SBMM in CoD and has patents for it).

It is all about finding the right build to clear areas quickly. I mostly run a sorceress. After trying out a lot of builds. With the sunder charms, single element sorcs now beat hybrid builds. My Tal set wearing, pure fire sorceress, clears areas extremely fast. It can also run all bosses at player 7, with only Baal taking some time to kill. Thats exactly what is wrong with diablo games. Youre forced to play certain way if you want to have any proper progress. I dont want to play meta, I dont want to play using guides, I want to play my way and if that makes the gaming suck be teadious; its problem with the game and developers. Unfortunately this is what d2 and d2r are. Sorcs are boring af unless you play something super expensive build. I play so little and have terrible luck with drops: I cant play those builds. Diablo 2 is a demanding game, but you clearly don’t need to play a lot to have an effective character.
All builds have budget versions that are almost as effective as BiS versions but much less time consuming to get.
Especially if you play in /player 1.
Just stop focusing on expensive items.

The fact that players / teams get to Hell on ladder reset day with basically nothing says it all. It will messy, with many a Death (certainly for me) but easily doable.

I must admit... initially, I was a weeeeeeee bit sour, but I'm grateful for any 45 lifer. *Cost 168 PGems*
Picked up a couple of decent small charms.
Throne of Destruction keeps delivering. What's curious is, I got this ethereal 4-holes Giant Thresher with a trapsin character equipped for 418% MF, and that's without a Gheed charm that I replaced with a traps GC for that run. That might have helped!
Edit: I just realized the trapsin's merc is equipped with an ethereal Giant Thresher base Infinity, that poorly rolled the first time at 256% ED, but -55% enemy lightning resistance. I'm wondering if I should give another try with the newly found poking stick.

Sick charms!
The Arachnid Mesh Belt dropped from terrorized Andariel, on my last run a bit ago. The Metalgrid also dropped from Andariel, just the run before the Belt. My first offline Metalgrid. The Lo rune dropped in the Arreat Plateau, just minutes after I began playing this morning.

Infinity is a very expensive runeword. I suggest you make another important runeword with 4 sockets. Also suggest that you spend the 2 Ber runes with other important runewords, if you don't have yet, such as Enigma and Chains of Honor. Focus in have the runewords, than you try reroll.
