You can use this formula to get another chance at a Magic Item of the same type. The ilvl = ilvl. This means to use 100% of the level of the input item.
If you're cubing for +skills Grand Charms, for example, you want to use Charms starting from late Nightmare, and not ones that dropped in Normal Blood Moor.
Be warned, however, that the chances of getting that on an Amulet is very low (considering everything that's available), and it might take a lot of tries. Rare items cannot be used with this formula to create new Rare items.

Any combination of 3 Perfect Gems - including Perfect Skulls - will work for this recipe. However, Perfect Amethysts and Perfect Rubies have better uses in crafting recipes (and a higher trade value) than other gems, and should not be used for this purpose.
If one is seeking to obtain "skillers" (Grand Charms with +1 to all skills of a particular tree), Grand Charms obtained in Nightmare Difficulty in the Flayer Jungle, Flayer Dungeon, Great Marsh, Swampy Pit, Spider Cavern, or Arachnid Lair are your best bets . As long as the zone is not terrorized, all Grand Charms dropped in these zones will be between ilvl 50-54, meaning they meet the minimum qualification for a +1 Skills prefix but do not yet meet the requirement for the next obtainable prefix ("Lucky" - 8-12%), which can be obtained starting at ilvl 55.
The iLVL of the resulting item is the same iLVL of the original item.
The best place to get Grand charms to reroll is Terror Zones with your character level 90. The normal monsters will be at level 92. The iLvl of the charm will be 91 and is possible to roll the highest affix level possible, Is Vita 41-45 to Life, this affix has level 91.
