Hit him several times with a bow and set Belt with prevent monster heal with visible lightning cue, Lower Resist Wand every 20 seconds or so, and light traps. It took me 8-10 minutes the other times and he never healed himself. As for the game name / pass you have to ask in the Party screen ("P" on usual commands) to see (eye icon) the game / pass.
Can't help on ladder, but if you're patient at some time someone will see that.
