You can only make them on Ladder. On non-ladder you can only use them (transfer them over from a previous ladder season). I believe this is correct after the Blizzard u-turn ^

The rune words that were added in 2.4 are still ladder-only.
The old rune words from before D2R are all available on non-ladder, even the ones that used to be ladder-only back then.
There seems to still be a lot of confusion around this, since sources for D2:LoD refer to the old rune words such as Insight as ladder-only, for obvious reasons (D2:LoD is still playable and there those rune words are still ladder-only).
