Achmel the Cursed is one of only two monsters - the second being

Lilith - that feature the unlisted and otherwise unobtainable "Poison Enchanted" unique monster modifier. This modifier increases Achmel's poison damage and resistance, gives his attacks and spells a high probability of applying a powerful poison, and grants an "aura" (or perhaps an invisible projectile) that periodically poisons nearby targets for a very long duration. Because Achmel's poison is so much more potent than its minions' and will quickly overwrite their effects, and the affix's +75% poison resistance only applies to the affected Unique, Poison Enchanted does not provide any meaningful benefit to Achmel's minions.

It is good to have

Sanctuary Aura on when facing this monstrosity. Sometimes it gets immune to physical damge and Stone skin modifier this can prove to be a problem and especially if you are melee character without minions or cannot be frozen equipment.
