Has anyone upgraded this to a Demon Head yet? I Just found one that's one point from perfect on the main 2 stats but a little low on the Enhanced Defense stat so I was wondering how much %ED, on average, upgrading one would gain

Some Trade is available for both rotw and LoD, Most rune , Perfect Gems is only in LoD side.

Upgrading doesn't reroll the %ED.
All it rolls is the new base def from the new item type's range. So 101-154 on a Demonhead.
Since hellwarden's does have %ED, you know your original base def was 87. You'd hence gain anywhere from 14 to 67 base def.
Your %ED will not change by upgrading. It'll merely be applied to a new base def value.
And as someone who upgrades everything, no, the appearance doesn't change (edit: though I just noticed you removed that question again):

Yeah I deleted the question shortly after I rolled the dice and found out for myself (defense went from 222 to 348).
