Maybe (sligh, slight chance) there are still some unrevelead secrets. Maybe not all possible cube combination of the standard and gems/runes/items etc have been tried out.

What about it opening a portal to something akin to the Cow-Level?
I saw speculations (for example by Mr LlamaSC) about a future 6th act in a coming expansion, so adding something like a new mini-region like the Cow-Level might not be too out of reach?
The new zone could be themed around a certain random aspect, for example the enemies being Undead or Demons or Beasts all, so that gearing against Undead/Demons/Beasts only would make sense.
Add to that a very small chance to get a Goblin-Layer like in Dibalo 3 would add additional spice.
The possibilities are nigh endless.
