The square outside the world?

I've seen quite a few off map locations in this game. Quite a few inside

The Countess tower as you

Teleport around and suddenly your in between two areas.

There's plenty of these in maggot lair, and I've even found one in cows. The "off location" was in a middle section of the map though (fenced off)
