You can't manually replenish ethereal javelins just like you can't manually repair ethereal items with durability, so they will run out eventually. The only way to replenish ethereal javelins is if they have the self-replenish suffix ("...of Replenishing" or "...of Propogation").

Titan's Revenge work because they have built-in self-replenishment, but generic magic/rare javelins don't always have this, it's a random suffix like so many others. For magic javelins you'd have to

Sacrifice the IAS suffix to have self-replenishment. Rares can come with both IAS and self-replenishment, I think.

True, but rares can't get the 3 jav's (Lancer's).

Rare javs can spawn up to +4 javs or +2 jav + 2all , but on rares you go for cruels mod 300+ ED and erep for jabbing, the skills do not matter there.

An example for GG ones would be 40ias/400ed/400ar/erep can also spawn skills ofcourse on top of that.

They have to spawn with thoose exact mods aswell to even beat

Eth Titans, 6/40s are for throwing, rares are for jabbing.
