1 - With the new expansion, can't we join friend game from different regions anymore (using the "join game" from the friend list context menu)?
2 - Friend List: Is there a way to search or sort/organize in a better way?
For me the list is ordered by status and then by login, so I have to visually scroll over all status to search for a user...
3 - why do some user appear online and in the middle of a D2 game on battle.net app, but appear offline on friend list?
3.1 - recently I have a user that is my friend (and was in a D2 game on battle.net app), but cannot find it on my friend list (searched for a long time... and never happened before not find a friend in the list...)
(after restarted D2 I could find it...)

1: Cross-region join has been bugged since RotW launched. Initially, it seemed to only affect steam to b.net and vice versa, by now it affects seemingly everything but apparently not all the time. Seems it might be tied to difficulty in some way as well (i.e. normal might work but hell might not).
2: Apart from the favorites bit, not that I know of. Though I also haven't looked into it much beyond that.
3: In-game friends list you mean? That seems to be suffering from some significant delays lately. Especially on Americas for some reason (for me at least). I.e. you log in, list is empty, after a (disturbingly long) time it'll start populating again in-game and match the b.net app one.
3.1: Probably ties in with the above.
4: You mean add as friend? Go to b.net app, use the 'add a friend' button above the friends list. There's probably some in-game command as well but I can't recall that, not using the in-game list for anything but friend joining myself. If you mean invite to a game then from what I recall that only works for online friends but actually haven't tried/used that in ages.
2 - i meant the friend list in the lobby.
On battle.net app I use favorite list to help (generally adding to favorite list everyone that I have a pendent trade with )
But see no way to improve the friend list in lobby
4 - user is already my friend, and i found it on the lobby friend list.
But if i try to invite it to a game, unless he is online in the lobby friend list, i receive an error messge ("cannot send invite" or something like that...)

Is there a configuration/extension/mod to appear offline in lobby even when you appear as online (and in a D2 game) on battle.net or when this happen it is a bug?
You have too many friends that listing cant handle it.
My current friend list is way worse than yours. It is completely broken. Not only the situation you see .
In my lobby list, if I scroll down, my list will keeps flashing and scan. not everyone will show in my friend list. The problem occur in yours will surely in mine.
My account have long long history, so does friend list, d2 d3....different friends from different time.
Please be aware, all my trade is in non ladder d2r. Not warlock.
I will pay (which is the most anti-consumer move ever) Blizzard to fix the lobbies/chats/interoperability between servers/platforms. Like this should all work out-of-the-box. But alas, we have a useless company piggybacking off a work of art that is D2.
Same thing was happening to my friends list on d2 (not BNet). People not showing up, wrong status displaying. I then removed about 400 people and it works just fine now
Will buy any 3 minor keys for an Ist - message me.
Will accept statues and keys instead of rune payments for my trades.

Your squelch list can be managed and edited from Top right dropdown > Account Prefs > Squelches.
