When an item has + to a skill that works for all classes (i.e., not just the class that has that skill naturally), the bonus is capped at +3. The difference is that if the item says "+X to Skill (Class Only)" you get the full bonus (as that class), but when it says "+X to Skill" without any mention of a class, the class that has that skill naturally gets max +3 and the other classes get the full bonus. It's for balancing reasons, I suppose.
The old Arreat Summit website calls this "oskill". It's been this way ever since this concept was invented in LoD. So a Sorceress can't wear partial Trang-Oul's to get a massive Fire Ball bonus, it's just +3. A Necromancer wearing 2 pieces gets +18 Fireball.
It's the same for CtA on a Barbarian, even if it rolls +6 BO they only get +3 from it (and the +1 to all Skills on it, of course, so total +4).
